Day Habilitation


Day habilitation services are services that assist or support individuals in developing or maintaining life skills and community integration. Programming is coordinated on-site and in the community to enhance opportunities to learn in a variety of environments. Day Habilitation services are designed to assist young adults to successfully transition to adulthood by increasing community integration and life skills.  Individualized goals and activities promote retention of skills and encourage essential competencies in the following areas:

  • Activities of daily living including self-care and physical care and assistance
  • Functional skill development
  • Mobility
  • Sensory integration
  • Language and communication development
  • Social/emotional and cognitive development
  • Gross and fine motor skills
  • Self advocacy


Who is eligible to participate in Day Habilitation services?

Site-based Day Habilitation service is offered through ChildServe for teens and young adults. They must be at least 16 years old and qualify for the Intellectual Disability Waiver (ID) to be enrolled for this service.  

How are referrals made?

Referrals can be made by a number of interested parties including members of the individuals care team parents/guardians, or the participant’s case manager. Referrals can be made to the respite and SCL service manager (school or summer breaks) or the day health manager at the Woodland Center (year round services).

How are services authorized and funded?

Day habilitation services are funded through the ID Waiver. There is no fee to the participant.

Services are authorized by the participant’s case managers. Typically there is a care team meeting to discuss service options, assessment of needs, identifying goals, and determining the frequency of services to be provided. A Notice of Decision is provided to ChildServe and the family stating how many units of services are authorized each month.

Where are services provided?

The program is currently offered throughout the year and during school breaks at the following ChildServe program locations:

  • The ChildServe Woodland Center
  • The ChildServe Respite Center  
  • Zion Lutheran Church


When are services provided?

Services are provided Monday-Friday with the following hours at each location:

  • The ChildServe Woodland Center – maximum of 8 hours during the hours of 6:30 a.m. – 6 p.m.
  • The ChildServe Respite Center - maximum of 8 hours during the hours of 8:30-4:30 p.m. primarily during school breaks
  • Zion Lutheran Church - maximum of 8 hours during the hours of 8:30-4:30 p.m. primarily during school breaks


Part time schedules are also available with minimum participation of 1 hour

I'm interested in enrolling - what happens next?

Once you have determined you are eligible for Day Habilitation services and are interested in pursuing this option through ChildServe, you can contact your case manager. Either you or your child’s case manager can contact ChildServe to begin the enrollment process.

A meeting will be scheduled to complete an assessment, develop care plans, and goals, and schedule of services for your child. The assessment may include evaluations provided by nursing, OT, PT, Speech and behavior support disciplines at ChildServe. Goals, objectives and identified support services will be established based on each individual’s needs and preferences. The plan will be implemented within 30 days of the initial assessment/care team meeting.

Staff provides care and activities that are monitored by the multi-disciplinary care team. Activities align with goals and objectives as outlined on participant’s individual plans. Specific areas of focus may include:

  • Activities of daily living including self-care and physical care and assistance
  • Functional skill development
  • Mobility
  • Sensory integration
  • Language and communication development
  • Social/emotional and cognitive development
  • Gross and fine motor skills
  • Self advocacy


Daily documentation on goals/objectives and other supports including the following:

  • Date, time, and location of services provided
  • Daily account of goals/objectives and supports provided
  • Daily summary/narrative that specifically highlights staff interactions, participant’s responses, progress on goals, and any unusual occurrences.
  • Documentation of all medications administered and/or specific treatments provided


The Day Habilitation program has a staffing ratio of 1 staff for every 3 individuals. Nursing oversight and support is available on-site or by phone. Child-specific training will be provided to all staff assigned to work with your child. Training will be provided by the individual’s family, the supervisor, and/or other staff who are familiar and have been trained on the individual’s plan of care.

Once enrolled and receiving services, you can expect the following on-going monitoring activity:

  • The individual’s supervisor will review all documentation on a weekly basis.
  • The case manager provides ongoing monitoring and authorization of services.
  • ChildServe will communicate with case managers and families at a minimum on a quarterly basis to discuss the individual’s progress.
  • ChildServe will participate in the annual care team meetings at the time waiver services are reauthorized for the new waiver year.
